Our business hours are 9:00 am to 6:00 pm, Monday through Friday. Please call for information on our extended hours.
- Driver’s License or valid identification.
- Insurance information.
- Completed New Patient Packet (for patients new to the practice).
- X-rays, MRI, CT Scan images (usually on disc).
- Written reports of all test results, including lab, X-ray, MRI or any other imaging test pertaining to your visit.
- List of medications you take including name, dosage, and frequency.- Driver’s license or valid identification. - Police Report. - Claim Number. - Declaration page. - Adjuster of Name. - X-rays, MRI, CT Scan images (usually on disc). - Written reports of all tests, including lab, X-ray, MRI or any other imaging test pertaining to your visit. - List of medications you take including name, dosage, and frequency.
We will verify your health insurance benefits prior to your visit, to determine if services will be covered. We recommend that you confirm your insurance is active prior to your visit.
