  • Our business hours are 9:00 am to 6:00 pm, Monday through Friday. Please call for information on our extended hours.

  • - Driver’s License or valid identification.
    - Insurance information.
    - Completed New Patient Packet (for patients new to the practice).
    - X-rays, MRI, CT Scan images (usually on disc).
    - Written reports of all test results, including lab, X-ray, MRI or any other imaging test pertaining to your visit.
    - List of medications you take including name, dosage, and frequency.

  • We will verify your health insurance benefits prior to your visit, to determine if services will be covered. We recommend that you confirm your insurance is active prior to your visit.
