Our business hours are 9:00 AM to 6:00 PM, Monday through Friday. Please call for information on our extended hours.

- Driver’s License or valid identification.
- Insurance information.
- Completed New Patient Packet (for patients new to the practice).
- X-rays, MRI, CT Scan images (usually on disc).
- Written reports of all test results, including lab, X-ray, MRI, or any other imaging test pertaining to your visit.
- List of medications you take, including name, dosage, and frequency.

- Declaration page.
- Adjuster of Name.
